After Death Occurs

In order to simplify the process of making funeral arrangements, PEOPLE’S MEMORIAL Funeral Cooperative has our funeral paperwork here. This is the paperwork your funeral director will use to complete your desired arrangements.

Death Certificate Worksheet
This form provides the necessary information for our staff so that they may promptly file the death certificate with the Health Department. No cremation or burial may occur until the death certificate has been signed by the physician and filed with the county.

Cremation Authorization Form
If choosing cremation, this must be signed by either the legal next-of-kin or the the designated agent.  If the next of kin is a group of children or siblings, then a majority of them must sign the authorization for the cremation to proceed.

Burial Authorization Form
You must also indicate whether or not you wish to have the body embalmed.  Embalming is not required in most situations.  This form must be signed by the legal next-of-kin or designated agent.  If the next-of kin is a group of children or siblings, then a majority of them must sign the authorization for the burial to proceed.
