In order to simplify the process of making funeral arrangements, PEOPLE’S MEMORIAL Funeral Cooperative now has placed our funeral paperwork online. This gives you the ease of being able to type the information directly into the forms in the comfort of your own home. Once you’re done, simply print them out, sign the forms and fax or email the completed paperwork back to us.
The Death Certificate Worksheet must to be completed regardless of whether you are choosing cremation or burial. This form provides the necessary information for our staff so that they may promptly file the death certificate with the Health Department.
For those choosing cremation, you must complete the Cremation Authorization Form. This must be signed by either the legal next-of-kin or the the designated agent. If the next of kin are a group of children or siblings, then a majority of them must sign the authorization for the cremation to proceed. No cremation may occur until the death certificate has been signed by the physician and filed with the county. You may also wish to check out our Urn Selection.
For those wanting Burial, the Burial Authorization Form must be completed. You must also indicate whether or not you wish to have the body embalmed. Embalming is not required in most situations. This form must be signed by the legal next-of-kin or designated agent.
