Online with a Credit Card
Our online application form allows you to enter your membership application electronically via a secure, encrypted link to our server. A credit card and e-mail address are required. Online memberships are honored as of the time submitted–24 hours a day 7 days a week and you will be given a membership number at the end of the process. An individual may join PMA up until the time the funeral home is notified of the death.

You may submit your membership application by fax with credit card information. Our fax number is 206-529-3801. Fax memberships are honored as of the time received, pending receipt of payment from your credit card.
In Person at our Office
If it is convenient for you to enroll in person, stop by our office, located on Capitol Hill in Seattle between Madison & Denny on 12th Avenue. Our address is 1801 12th Ave Suite A (map/driving directions). We are open Monday-Friday from 9 AM to 5 PM. You may pay by cash (exact amount only), check or credit card. We are unable to make change for cash transations.
Membership applications by phone are accepted during business hours if death is imminent. A credit card number will be needed. The number for our membership office is 206-325-0489 or outside the Seattle area: 1-866-325-0489. Office hours are Monday-Friday 9-5pm.
